Chalazion, Stye, and Hordeolum

A chalazion or stye (hordeolum) is a small bump that develops on the eyelid due to inflammation or blockage of an oil gland. These glands—called meibomian glands—help keep the eye surface lubricated. When one becomes clogged, oil builds up and forms a tender lump under the skin.

A stye (or external hordeolum) is typically caused by a bacterial infection at the base of an eyelash follicle, leading to a red, painful swelling. A chalazion, on the other hand, usually develops more slowly and tends to be less painful, arising from a blocked oil gland without active infection. Sometimes, a stye can evolve into a chalazion if inflammation persists after the infection resolves.

These eyelid bumps are quite common and can affect anyone, often triggered by factors such as blepharitis (eyelid inflammation), oily skin, hormonal changes, or poor eyelid hygiene.

Symptoms

Patients may notice:

  • A red, swollen bump on the eyelid (upper or lower)

  • Tenderness or soreness, especially in early stages

  • Mild tearing or irritation of the eye

  • Occasionally, blurred vision if the swelling presses on the eye

  • Cosmetic concern or discomfort when blinking

Most cases are harmless, but persistent or large lesions may become bothersome or occasionally lead to secondary infection.

Treatment Options

In most cases, treatment is conservative and simple:

  • Warm compresses several times a day help open the blocked gland and promote natural drainage.

  • Gentle lid massage can aid in resolution.

  • Topical or oral antibiotics may be prescribed if there is active infection or associated blepharitis.

  • Anti-inflammatory drops or ointments may help reduce swelling and tenderness.

If a chalazion or stye does not improve after several weeks, or if it becomes chronically inflamed, surgical management may be recommended.
